    Charity Nomination Form

    By submission of this form, the member is verifying the information contained is correct to the best of their knowledge.
    • I am submitting this form at least one week prior to the meeting.
    • ​I  am a current member in good standing of 100 Women Who Care, Troy. All of my contributions are current.
    • ​I have attended at least one meeting prior to the upcoming meeting.
    • I will arrive early to to check in before the meeting starts. ​
    • I have verified the charity is a 501(c)3. Search via the IRS site.
    • I am prepared to provide a 5 minute presentation to members and spend a maximum of 5 minutes to respond to questions.
    • I understand that I must be the person to present and may not have a representative present for me. Only members may nominate and present for a charity.
    • I have reviewed the Presentation Guidelines Document (WORD)
    • I  understand that if the information contained is incorrect, not submitted on time or the charity is not able to be verified, the charity will  not be eligible for presentation.
